This form should be submitted before you complete any part of the enrollment forms to make sure we will be able to admit your child and have room in your child's grade level. We will contact you after you submit this form to let you know if your child has been admited. You should then print and complete the enrollment documents on the Enrollment page and bring them to our registration desk. If there is not room at this time in your child's grade level, we will add your child to our waiting list for possible future enrollment.
